TO ADULTHOOD PROGRAM
Identity First offers holistic and personalised support to help teens with disabilities transition from school to adulthood with confidence and independence. Our social worker begins by assessing functional capacity and independent living skills, then works with the teen to identify goals for a tailored support plan. We connect teens with the necessary services (including support workers) and provide ongoing guidance in key areas, including:
